This narrated video illustrates the surgical endoscopic technique to remove an attic cholesteatoma with extension medial and posterior to the ossicular chain. The endoscopic transcanal atticotomy allowed excision of the cholesteatoma with preservation of the ossicular chain. The scutal defect was reconstructed with a composite tragal graft. In addition, the utilization of piezosurgery in endoscopic ear surgery allows transcanal atticotomy with a blood free surgical field, limited bone removal and minimal risk of soft tissue injury.
