There is some problem either in the stomach, intestine or colon. Blood in stools with pain in the upper abdomen usually indicates an ulcer. Ulcer can be in the stomach or small intestine what we call as the duodenum. These are the primary reasons where we get pain and blood in the stools. Blood in stools is most commonly a problem related to the colon, either in the rectum, piles or fissures. Other problem can be ulcers or rarely it can be tumours. But in young females usually it is piles or fissure, originating from the colon. Rarely it can be a big ulcer in the stomach causing bleeding and pain. Other can be some pancreatic related problems wherein there may be bleeding from the vessels what we call as the splenic vessels, they open up and they bleed. That is the reason why we get frank blood in the stools. But by enlarge blood in stools indicate colon and rectum.
