Aletheia will summon other (Neo-)GUILT to protect itself from harm. Everytime you destroy another GUILT, you have a chance to inject the serum into Aletheia's core. Once you've gotten rid of them all, the eye will be defenseless for a period of time. Then, you will have to 'sever the chains of fate' (sever the veins). You do not want to touch the veins when it's red, as Aletheia will recover and you'll have a big vital drop. When you get to the last part of the operation, you will have to use the Healing Touch to slow down the veins of changing color so you can sever them.
