For these playthroughs, I'm using Darkplaces with all of these modifications located here: (see my autoexec for exact bloom/reliefmapping/etc settings)

Slightly exaggerated Bloom (settings in autoexec.cfg)
Real-Time World enabled, RT World lightmaps 0
Relief mapping enabled (in autoexec.cfg)
High resolution water enabled (in autoexec.cfg)
Motionblur and damageblur (in autoexec.cfg)
Shadowmapping enabled (r_shadow_shadowmapping 1)
The monster models and maybe a few other things are the same as stock Quake because I couldn't find any good replacements.
