They try hard to save themselves with all those defense mechanisms like puffing air or water into their stomach or covering themselves with tetrodotoxin. Although not all puffers are poisonous, flesh of some puffers do not contain toxins and that makes them a desirable delicacy in Japan and North America.
If this fish is cooked improperly, some toxin from the organs of the puffer fish can enter the dish and cause death or severe life threatening symptoms.Tetrodotoxin is 100 times stronger than cyanide.
It is much better and safer to just enjoy watching it. Just look at the white teeth this fish uses to crack small crustaceans open! Probably best to just leave this fella alone.
