PTTD, or posterior tibial tendon dysfunction, is the most common cause of painful flatfoot in adults.
the tibialis posterior is a muscle behind the shin bone and transitions to the tendon down around the ankle. It attaches below the arch and is the main tendon that holds up the arch.
If this tendon weakens and degenerates, the foot will flatten, and often become painful. Minor foot changes may eventually lead to total arch collapse, painful foot deformity, or even tendon rupture.
Tendon weakness rarely happens overnight. It is classified as a progressive overuse injury where degenerative tendinosis occurs rather than inflammation.
The more strained the tendon becomes, the more damage it accumulates. This can occur in a child. In an adult, it is termed adult-acquired flatfoot deformity.
The pain associated with this chronic injury is multi-faceted and can be a burning, shooting, tingling, and/or aching pain. It could worsen with activity or as the day goes on. You may even notice that the foot has deformed before there is even any pain.
