PFUDD causes neurogenic or vasculogenic impotence,sometimes both.A penile doppler is done to document the velocities in dorsal penile arteries.Patients with poor flow in dorsal penile arteries can be candidates for Penile revascularisation. This is done by anastomosing the bilateral inferior epigastric arteries to the dorsal penile arteries or vein or both.The results are complimentary with use of sildenafil and PDE5 inhibitors.We present a video of the penile arteries revascularisation. This surgery was performed by Dr Sanjay Kulkarni.We recommend this surgery for patients prior to urethroplasty in case flow is poor in both dorsal penile arteries.Technically it would improve the success of urethroplasty by supplying good blood flow to the transacted bulbar urethra and bulbs membranous anastomosis after urethroplasty.
