Pelosi, III, MD, cosmetic gynecologist and cofounder of the International Society of Cosmetogynecology (ISCG), outlines the steps for effective labia minoraplasty (labiaplasty, labia minora reduction). The significance of properly identifying excess tissue in the region of the clitoral hood (prepuce) is essential to acheiving superior cosmetic results. Frequently this tissue is a reduplication of the anterior labia minora and needs to be contoured with specific techniques. Other areas of detail include the management of the frenulum (suspensory ligament) of the clitoris when it is elongated and the management of circumferential elongation of the labia minora above the posterior labial commissure (fourchette). Dr. Pelosi illustrates his methods of beveled linear labiaplasty and "horseshoe" labia minoraplasty and how he adjusts them to achieve different profiles of the labia minora safely and effectively. Also illustrated are the biomechanical limitations of wedge labiaplasty and why techniques of wedge labiaplasty are more prone to failure. Local anesthesia is more than adequate for labiaplasty and the innervation of the labia minora and clitoral region is addressed in detail. Complete healing time is approximately six weeks.
