The best approach for repair of large inguinoscrotal hernias and management of the distal sac is a subject of debate even today. We consider a large inguino scrotal hernia when the distal sac goes deeply into the scrotum. Attempting to completely dissect the sac in these circumstances risks lesion of the cord structures and orchitis. The problem with an unattended distal sac is the formation of large and sometimes cumbersome hematomas ,seromas or pseudo hidroceles. Repetitive drainage and occasionally surgery may sometimes be necessary in these cases.
In this video we describe a technique we have been using for more than a decade that addresses the problem of handling the distal sac.
