Educational video describing supination - external rotation fracture injury to the ankle. Supination-external rotation fracture is the mnost common type of ankle fracture. Clinically the fracture can be type II an isolated fibular fracture or type VIthat includes a fibular fracture in addition to a medial malleolus fracture or deltoid ligament injury. Sometimes it is difficult to differentiate between type II and type VI fractures and stress views may be necessary. Supination-external rotation is up tp 70% of ankle fractures. Stage 1 : disruption of anterior tibiofibular liagmetn with or without an associated avulsion fracture at its tibial or fibular attachment. Stage 2: spiral fracture of distal fibula (from anteroinferior (A1) to posterosuperior (PS)) at, below or above syndesmosis. Stage 3: disruption of posterior tibiofibular ligament or fracture of the posterior malleolus. Stage 4 : fracture of the medial malleolus or rupture of the deltoid ligament. The fibular fracture could be located at, below or above the level of syndesmosis. The fibular fracture is a coronal fracture that runs from anteroinferior to posterosuperior.
